In December, 2004, a 63-year-old man was referred to us for investigation of a 20-year history of syncope preceded by intense laughter. He had no medical history except for well-controlled type 2 diabetes. He stated that he had fainted approximately ten times during the past 20 years, and on a few occasions felt that he was going to faint when coughing.
